For 140 years, the Society of SDVP has helped Greater Cincinnatians in need, regardless of race or creed, through person-to-person assistance with food, shelter, utilities, clothing, and essential medicines. Thirty percent of SVDP’s pharmacy clients are uninsured and without a medical home (a primary care provider that is visited on a regular basis for preventative and urgent care). Since the recession, SVDP has had to adapt to having fewer financial resources to help this underserved population, despite an increased demand during these economically challenging times.
James Banks, medical student at the University of Cincinnati and founding member of Global HEED, spearheaded a free clinic through the University of Cincinnati Academic Health Center and his local IHI Open School Chapter. James worked closely with students and faculty at UC to develop UCIC which features a robust self-management support program for chronic disease management. UCIC engages its patient in individualized action plans to address specific risks to their health and improve their overall quality of life. Focusing on community engagement and capacity building, this project provides SVDP’s clients with the knowledge and skills to transform their own lives using what resources they have.
